NOTICE IS HEREBY GIVEN that Applicant, ZP Disposal Systems, LLC, P.O. Box 1289, Kingfisher, OK 73750-7084, is requesting an exception to OAC 165:10-5-7(f)(1)(C).

NOTICE IS FURTHER GIVEN that the Jet SWD #1, is a commercial saltwater disposal well, located in the NE/4 NE/4 SE/4 NE/4 of Section 26, Township 20 North, Range 4 West, Garfield County, Oklahoma, API No. 047-22334, authorized by Permit No. 1006160149, dated July 14, 2010, for disposal into the Pawhuska, Endicott and Lovell Formations from 2,945 feet to 3,735 feet at the rate and pressure of 2800 BWPD at 1000 PSI Surface.

NOTICE IS FURTHER GIVEN that the Jet SWD #1 well failed its most recent mechanical integrity test. OAC 165:10-5-7(f)(1)(C) requires that the well must be brought into compliance within ninety days after discovery of the problem.

NOTICE IS FURTHER GIVEN that Applicant requests an exception to OAC 165: 10-5-7(f)(1)(C), thereby authorizing additional time beyond the ninety days to evaluate their repair options further, divesting of the disposal well, or proceed with plugging operations.

NOTICE IS GIVEN that written objections may be filed with the Oklahoma Corporation Commission within fifteen (15) days of publication of this notice. Written objections, if any, shall be mailed to the Oil and Gas Conservation Division, Underground Injection Control Department, Will Rogers Memorial Office Building, P. O. Box 171, Oklahoma City, OK 73101-9918, or 2401 N. Lincoln Blvd, Oklahoma City, OK 73105.
