You will hereby take notice that the State of Oklahoma, by and through Tommy Humphries, Garfield County District Attorney, has filed a Deprived Child Petition, in and to the above named child. An initial appearance as to Termination of Parental Rights for the unknown father, will be conducted on the 8th day of January, 2026, at 1:30 P.M. Further, that as the unknown father of E.A., you have the right to appear and to have an attorney represent you at the hearing.

FAILURE TO RESPOND TO THIS SUMMONS OR TO APPEAR AT THIS HEARING OR ANY FUTURE HEARINGS MAY RESULT IN YOUR CHILD BEING FOUND DEPRIVED, THE LOSS OF CUSTODY OF SAID CHILD(REN) AND MAY RESULT IN THE TERMINATION OF PARENTAL RIGHTS TO SAID CHILD(REN). IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED, YOU MAY LOSE ALL LEGAL RIGHTS AS A PARENT TO THE CHILD OR CHILDREN NAMED IN THE PETITION OR MOTION ATTACHED TO THIS NOTICE. THE DUTY OF THE PARENT TO SUPPORT HIS OR HER MINOR CHILD WILL NOT BE TERMINATED EXCEPT FOR ADOPTION AS PROVIDED BY PARAGRAPH 3 OF SUBSECTION B OF SECTION 1-4-906 OF TITLE 10A OF THE OKLAHOMA STATUTES.
